Fitness and wellness
Form helps you track workouts and nutrition and review wellness trends. It does not provide medical advice, diagnosis, or treatment. Coach answers and nutrition estimates may be incomplete or incorrect. Review entries and proposed changes before saving them.
Choose exercise and nutrition decisions suited to your circumstances. Consult a qualified healthcare professional when you need medical guidance. Do not use Form for emergencies.
Form Plus subscriptions
Optional Form Plus features are offered through monthly and annual subscriptions. The app shows the included features, localized price, and billing period before purchase. Apple charges your Apple Account when you confirm.
Subscriptions renew automatically unless canceled at least 24 hours before the current period ends. Apple may charge for renewal within 24 hours before that period ends. Price changes follow Apple’s notice and consent rules.
Manage or cancel your subscription with Apple. To restore access, use Restore purchases in Form’s Account & Settings with the Apple Account used for purchase. Request a refund from Apple, subject to Apple’s policies and applicable law.
Deleting the app or your Form account does not cancel an Apple subscription.
Your information
Read the Privacy Policy for information about accounts, Apple Health, Coach, service providers, retention, and your choices. Optional AI and Health sharing require separate permission.
You can export your records or delete your account in Account & Settings.
