Restore purchases in Settings using the Apple Account that purchased Form Plus. Manage or cancel your subscription with Apple. Deleting Form or your account does not cancel a subscription.
Your data
Request an export or delete your account from Account & Settings. See account deletion instructions.
Change Health sharing in Settings → Apple Health. Manage read permissions in Apple Health. For privacy requests, email privacy@form-fitness.app.
Report a problem
Include your iPhone model, iOS version, Form version, and the steps that led to the issue. Please do not send passwords, verification codes, payment details, or health records.
