This Privacy Policy explains how Serplexity LLC (“Serplexity,” “we,” “us,” or “our”) handles personal information in connection with the Form Fitness mobile application, website, and support services (together, “Form”). Form helps clients access services and training information provided by their personal trainer or fitness business.

2. Information we handle

Depending on how Form is configured and used, we may handle:

• Account and contact information, including your name, email address, account credentials, and identifiers.
• Training and service information, such as assigned programs, workout activity, progress entries, assessments, bookings, schedules, and information shared with or entered by your trainer.
• Support communications, including messages and information you send when requesting help or exercising a privacy right.
• Device, usage, and diagnostic information, such as device and operating-system details, IP address, app interactions, crash reports, and performance data.

Your trainer or fitness business may collect information from you directly and make it available through Form. Their handling of that information may also be governed by their own privacy policy.

3. How we use information

We use information to:

• Provide, maintain, and secure Form;
• Authenticate users and keep account information available;
• Display programs, workouts, progress, and schedules;
• Process and manage bookings and service requests;
• Respond to support and privacy inquiries;
• Understand performance and improve reliability; and
• Comply with law and protect users, Form, and Serplexity.

4. How we share information

We may disclose information:

• To your trainer or fitness business so they can provide and manage the services you receive through Form.
• To service providers that help us host, secure, analyze, maintain, and support Form, subject to appropriate contractual safeguards.
• For legal and safety reasons when reasonably necessary to comply with law, respond to legal process, or protect rights, safety, and security.
• In connection with a business transaction such as a merger, acquisition, financing, or sale of assets, subject to applicable law.
• At your direction or with your consent.

5. Retention and security

We retain personal information for as long as reasonably necessary to provide Form, fulfill the purposes described here, meet legal and accounting obligations, resolve disputes, and enforce agreements. Retention periods vary based on the type of information and how Form is used. When information is no longer required, we take reasonable steps to delete or de-identify it.

We use administrative, technical, and organizational safeguards designed to protect information. No method of storage or transmission is completely secure, so we cannot guarantee absolute security.

6. Your choices and rights

Depending on where you live, you may have rights to access, correct, delete, restrict, or receive a copy of personal information, or to object to certain processing. You may update some information through your trainer or fitness business. To submit a privacy request, email us from the address associated with your Form account. We may need to verify your identity before completing the request.

If a child uses Form, their parent or legal guardian—or the organization responsible for the account—should contact us to exercise privacy rights on the child’s behalf.

7. International use

Information may be processed in the United States and other countries where Serplexity or its service providers operate. Those countries may have different data-protection laws. Where required, we use appropriate safeguards for international data transfers.

8. Changes to this policy

We may update this Privacy Policy as Form and applicable laws change. We will post the updated policy here and revise the effective date. If an update materially changes how information is handled, we will provide additional notice where required.
